How do I steam presses a shirt?

The problem is the first step: the shoulder yoke. Because of the thickness of the collar, it never steam presses the shoulders very well at all. And how do I get rid of the crease it leaves?

to iron the back yolk--fold the shirt where the yolk meets the back along the steam then you can lay it flat & iron it
